Community Nursing
First Community Health and Care
This NHS team helps people who cannot leave their home. They support people who are very ill or near the end of life.
You can:
- ask for help yourself
- ask a carer to do this for you
The team also takes referrals from GPs and other professionals.
They can help with:
- taking medicines, including diabetes care
- caring for wounds and preventing sores
- leg ulcer care
- end-of-life support
- catheter and continence care
- giving antibiotics through a drip
- removing chemotherapy lines
- blood tests
Time
7 days a week, 365 days a year, 8am to 11pm (if any urgent overnight nursing care is required your Community Nursing team will provide details of support and contact details)
