Community Stroke and Neurological Rehabilitation and Early Supported Discharge
First Community Health and Care
This NHS service offers specialist assessment and advice for people living with neurological conditions such as stroke, multiple sclerosis, and motor neurone disease. Support is also available for carers.
The team can help with:
- mobility
- communication
- accessing the community
- returning to work
- eating and drinking
- washing and dressing
- managing household tasks
- memory and thinking skills
- adjusting to disability
You will be seen at home or at the Oxted Therapies Unit. Therapists will arrange appointments and may set up a programme for a Rehabilitation Assistant to carry out.
The service works with you to assess your needs and agree goals. These may focus on daily living skills, physical recovery, communication, and memory or concentration. The aim is to help you stay as independent as possible in your community.
To use this service, you must:
- be aged 18 or over
- be registered with a local GP
- need short-term, goal-based support for a neurological condition
Referrals can be made by GPs, local hospitals, or inpatient rehabilitation services.
