This service is for stroke survivors, including those who have had a transient ischaemic attack (TIA or mini-stroke), as well as their families and carers.
The service offers:
- coordinated support throughout the stroke journey
- home visits and regular phone calls
- emotional support
- tailored information
- help with accessing community-based support
- support for carers and family members
- signposting to other relevant organisations
A monthly peer support group also meets on the first Wednesday of every month at:
