This service is for stroke survivors, including those who have had a transient ischaemic attack (TIA or mini-stroke), as well as their families and carers.

The service offers:

  • coordinated support throughout the stroke journey
  • home visits and regular phone calls
  • emotional support
  • tailored information
  • help with accessing community-based support
  • support for carers and family members
  • signposting to other relevant organisations

A monthly peer support group also meets on the first Wednesday of every month at:
