District nurses play a crucial role in the primary health care team. They visit people in their own homes or in residential care homes, providing increasingly complex care for patients and supporting family members. Your doctor, social worker or hospital will refer you if you need home nursing care. You can also refer yourself by emailing or calling.

District nurses:

• assess the healthcare needs of patients and families
• monitor the quality of care they're receiving
• be professionally accountable for its delivery

They can also offer palliative/end of life care at home.
